I hereby relieve Somerset County Recreation & Parks, its staff, instructors or volunteers from any responsibility for any bodily injuries, etc., that may incur by myself or child as a result of participation in this program and/or activity.
By typing my electronic signature I am also aware of the program rules and will comply to those rules, including following directions given by the staff, coach and/or official/referee. Failure to obey these rules may result in my dismissal from the program.
